For neurological assessment - children 1-3 years of age, I know we must assess verbal, visual, and tactile stimuli but for under one are we just checking fontanel?

Just to clarify... For Fontanel - that of course only applies under the age of 1.
You still have to chart on Visual Auditory Tactile for not only 1-3 years of age, but for a non-communicating child as well. This is easy enough to do for a 8 month old baby too, when they track light, turn towards sound, and when they move their hand when you gently stroke their hand, or when they grab your finger. If you are unable to assess VAT, you will definitely be able to "determine patient's ability to recognize familiar people or objects"... the patient inclined to go toward mother when someone else is holding the child or holding their hands up only towards mom to pick them up...
Be thorough with your charting... Don't be limited. For example, you asked the child who is not yet 3 and they correctly identified what their name is and where they are..you can also mention that in your notes.....If you are unsure whether or not you can mention it because you are charting on VAT for sure, then write in parenthesis or write in other observations.
