NONE of your interventions should be an assessment. I'm putting together handouts for various diagnostic labels... see excerpt below.
Acute Pain • Medicate patient (if assigned) • Request primary nurse to medicate patient • Provide distraction with conversation (tv, book, pediatric distractions – pacifier, playing, singing, toy, cuddling) • Assist with repositioning • Encourage use of PCA pump when in pain • Encourage guided imagery • Apply heat/cold if assigned • Provide back rub • Dim lights (headache)
Impaired Physical Mobility • Assist patient with ambulation in the hallway X 1 during PCS (or designated activity on the Kardex) • Provide patient with walker (cane) for safe ambulation • Assist patient with range of motion exercises to maintain level of mobility • Maintain on CPM while in bed
