Question: During the PCS, if I have to flush the IVAD, where do I record it?
Answer: When you first go to the patient's room you will do the HIPPI COW (mnemonics for the 20 min checks) where you would glove up and check the IV site for edema or other signs of infiltration. You would then take your gloves off, and wash your hands.
At this time as a part of the 20 min checks, don't forget to open to the "PCS RECORDING Form" and write in the box on the farthest right column labeled Intermittent venous access device maintenance. You will then chart "No edema" next to Edema.
This step has to be done during 20min.
Your flushing the IV site can wait till later. When you go get the meds, you can bring your flush at that time and administer.
DON'T FORGET TO:
1. Check the ID against the MAR before giving the meds.
2. Glove up before checking the IV site again right before the flush. REMEMBER IGI ID GLOVES IV SITE Before any access to the IV
3. Clean the IV port before accessing with the flush
4. Aspirate for blood return
5. Flush ---- GOES IN THE SHARPS!!!
6. Close the clamp if any
7. Take gloves off
8. Wash your hands
9. Chart on the MAR
AND
10. Come back to the "PCS RECORDING FORM" and chart next to Flush (type and amount): __?_ml Flush 0.9% Sodium Chloride or just Normal Saline is fine.
So, when you look at the PCS RECORDING FORM... I want to you think about it as....
SOME things on this page need to be charted on in the first 20 min of the start of the implementation phase. BUTTTTTTT some of the things you need to come back to before turning in your documentation in to the CE.
Also don't forget to put 1 mL in the Parenteral box if you are assigned intake and output. All of this information is in the Fluid Management videos.
